3rd header level with too much space in the table of contents
As per the attached image, header levels 1 and 2 are lining up very nicely in the table of contents. However, level 3 seems to insert a long row of dots between the numbering and the text. This causes these items to not line up correctly on the right, and makes the appearance quite bad. What is caus...

Stop 'Table of Contents' from showing itself in the Table of Contents
When I format the words 'Table of Contents' as a heading, it will then show in the document structure, as per the left section of the attached image, where the left side of the red arrow is pointing. I do want it to show in the structure. That makes it a clickable link to quickly jump to it. However...
